We provide referrals to specialists, order imaging, and can complete medical forms. We are committed to providing detailed and thorough medical reports for all patients.

Patients then meet with our team of physicians and referral appointments with specialists are completed as needed.

Clinical Records & Forms
Thorough notes and reports are completed along with any other forms or reports required (including but not limited to OCF3, CCP Forms, ODSP and LTD Applications, and Trillium Applications).
