Three steps from referral to care
- Send the referral. Use our two-minute online form, email referrals@circlebaymedical.com, or send an OCF-1 or police report. Upload records securely if you have them.
- We take it from here. We contact the patient, gather records and book a virtual visit, typically within one week.
- Care, records and forms. The patient sees our physician-led team. Clinical records and forms follow, along with specialist referrals, imaging and follow-up visits where needed.
A treating practice, not a retained assessor
We are the patient's treating medical team. Our duty of care runs to the patient, and our clinical records document real care, written by the clinicians providing it.
